Forms are fillable, but must be opened in Adobe Reader. They will not be fillable when opened in your browser.  Please download forms to your computer and open in Adobe Reader.

Download instructions:

• Click form to be downloaded

• Click download icon in the top toolbar of your browser

• Click on the first file - it will be a series of letters and numbers

• Open with Adobe Reader and save to your computer, renaming with your name for Jr Dues or club name for Senior dues

• Fill out form and save

• Senior Dues form - Attach to an email and send to Lucinda Lear, IFMC Treasurer at lmlear@mchsi.com

• Jr Dues form - Attach to an email and send to your local treasurer
